Docs: correct ADR README references and CHANGELOG/ADR-0006 date ordering #83

Audit findings N11 + N12 + N13 — verified.

Problem

• docs/adr/README.md:36 cites both "Issue docs: clarify provider as an integration role #10" and "PR docs: clarify provider as an integration role #10" — impossible under GitHub's shared issue/PR numbering; one reference is wrong.
• docs/adr/README.md:4 link text says spec/lpp-v1.md but the spec is split (target ../spec/README.md is correct).
• CHANGELOG.md releases resolveIds as 1.7.0 dated 2026-10-10 while docs/adr/0006 records the decision date 2026-10-11 — the recorded decision post-dates the shipped feature.

Acceptance criteria

• Each reference/date corrected or annotated so the record is self-consistent.
• Review-based acceptance.
