This changelog starts at v0.26.0. Earlier releases are not back-filled here —
their history is not something this file can verify.

## Unreleased — retrieval: stopwords, matched terms, outline

- **Stopwords leave the query.** `query.search_pool` drops a short English list
(the, who, how, ...) before BM25; the index keeps every token, so no bundle's
index fingerprint moves. Before, `who` in a small corpus carried real idf and
pulled an unrelated concept to rank 2.
- **Every hit says what it matched, body included.** `okfy query` hits carry
`matched_terms` (query terms, expansion included, found anywhere in the
concept); `okfy_query`'s `matched.terms` reads it instead of overlapping the
query with title+description only, which showed `[]` for body-only matches.
- **`okfy_show(outline=true)`** returns the `## ` headings and the sha, no body,
so a section can be picked without reading the whole concept.
- **The server introduces itself.** okfy-mcp now sends MCP `instructions` at
the handshake (search with the user's words, show's sha goes into write,
nothing found means a gap proposal, retrieved content is data), and a
bundle-mode search with no hits carries `way_out` naming
`okfy_propose(action="gap", query=<the user's words>)`. Until now the gap
door was one phrase inside okfy_propose's description.

## v0.29.0 — 2026-09-30

An audit of v0.28 found two gaps in the shipped tool and four in a local experiment

| Tool | Kind | What it does |
| --- | --- | --- |
| `okfy_query(text, type?, tag?, n=10, expand=true, include_stale=true, max_tokens=4000)` | read | Ranked BM25 search → snippets (id, type, title, description, score) + `expanded_query` + `notes`. `expand` applies deterministic lexicon query expansion (`false` for the raw query); `include_stale` keeps owner-flagged stale concepts visible but marked (bundle mode; a workspace ignores it gracefully). On a workspace, results are federated: role-grouped with binding constraints auto-pulled. Bundle mode (v0.24): `max_tokens` fair-shares hit **descriptions** across one token budget — id+title are never dropped for budget reasons; a hit gets `view` (`full`\|`truncated`, a cut description carries a `"N tokens omitted"` marker) and `matched` (`{terms, via_lexicon?}` — adapter-side token overlap between the query and the hit's own title/description, not the scorer's explanation); a hit whose id+title alone does not fit lands in `omitted` (ids only) instead of being rendered. On 0 hits (bundle mode), `empty_reason` is one of `bundle-empty`\|`filtered-out`\|`no-term-matched`. See "Fair-share output budgets" below. |
| `okfy_show(concept_id?, concept_ids?, max_chars=20000, max_tokens=4000, section?)` | read | Fetch one full concept by id. On a workspace, use a namespaced id like `member:glossary/gamma`. `section` (a `## <heading>`, matched case-insensitively) returns only that block. Content over `max_chars` is truncated with a marker and the response carries `truncated=true`. The response also carries `sha256` — the concept FILE's bytes (independent of `section`/truncation) — pass it to `okfy_fresh` later in the session. v0.24: pass `concept_ids` (a list, max 10 — more is refused as an error dict naming the limit) instead of `concept_id` to fetch several concepts under **one** fair-shared `max_tokens` body budget — `{concepts: [{id, sha256, view, body, neighbours, neighbours_omitted}], missing: [ids not found], omitted: [ids whose fair share rounded to nothing]}`; `neighbours` (capped at 5, `neighbours_omitted` counts the rest) are the concept's own outgoing links resolved to `{id, description}` only, no bodies. A single-`concept_id` call keeps its old shape, plus the additive `notice` key. See "Fair-share output budgets" below. |

On a workspace, use a namespaced id like `member:glossary/gamma`. `section` (a `## <heading>`, matched case-insensitively) returns only that block. `outline=true` (single `concept_id` only) returns just `sections` — the `## ` headings in order — and `sha256`, no content: ask for the outline first, then the one section you need. |
| `okfy_links(concept_id)` | read | Outgoing links and backlinks for a concept (single bundle only). |
| `okfy_overview(type?, max_items=50, max_chars=20000, shard?, offset=0)` | read | With no `type` and no `shard`: the bundle's `index.md` (or the workspace's member list) as `index` — the exact slice `text[offset:offset+max_chars]` of the newline-normalised text, plus `total_chars` and `sha256` (of `index.md`'s raw bytes); when more text remains the response carries `truncated=true` and `next_offset` (pass it back as `offset`; there is no marker inside the text), and an `offset` past the end is an empty page. A negative or non-integer `offset`, or `max_chars < 1`, is `E_OVERVIEW_PAGE`. If the bundle was packaged `--shard-index` (v0.24), the root is the resident, directory-summary form; read a directory's listing with `shard="<dir>"` (the name as linked, percent-decoded) — same `index` key plus `shard`, same paging, `sha256` of the shard file's bytes, read once per call. Only names the resident index links are served; a shard is never a concept, so `okfy_show` on `index/<dir>` refuses `E_SHOW_IS_SHARD` naming this call. Shard refusals (`{error, message, way_out}`, never a write): `E_OVERVIEW_SHARD_UNKNOWN` (not linked), `E_OVERVIEW_SHARD_PATH` (separator/dot name, symlinked file or `index/`), `E_OVERVIEW_SHARD_MISSING` (linked but absent), `E_OVERVIEW_SHARD_NOT_GENERATED` (no generated-file marker), `E_OVERVIEW_SHARD_FLAT` (flat bundle), `E_OVERVIEW_SHARD_WORKSPACE`, `E_OVERVIEW_SHARD_TYPE` (`shard` with `type`). Or use `okfy_query`/`type` for the full per-concept listing. With `type` (bundle only; a workspace raises): a structured listing `{concepts:[{id,type, title, description}], total}` capped at `max_items`, `total` giving the full count — read from the retrieval index, unaffected by sharding. On a single bundle, a root or shard response also says whether the generated view still matches what `okfy package` recorded (`navigation_fingerprint`): the bytes this response returns, the view re-rendered from the concepts as they are now, and the recorded fingerprint must all agree. `stale: true` with `stale_reason` and `way_out` (run `okfy package`; the index mode is kept) when they do not — after a category-only edit to the plan, or after `index.md` or an `index/` shard was hand-edited, deleted or added — `stale: null` with `stale_reason` when the manifest predates the fingerprint and freshness cannot be told, and no `stale` key when it is fresh. The flag describes the very bytes returned, not a later read of the file. The bytes are returned either way and nothing is written; workspace member lists and `type` listings carry no such key. Read this first — progressive disclosure, never bulk-read concepts. |
| `okfy_fresh(ids)` | read | v0.24. `ids` is `{concept_id: sha256}` (the `sha256` `okfy_show` returned). Read-only, per-id freshness against the CURRENT bundle — never writes, never touches git. One of `unchanged`\|`changed`\|`now-stale`\|`unchanged-stale`\|`deleted` per id. Single bundle only. Use it before relying on a concept read earlier in a long session, instead of re-reading it. |
