docs(sdd): archive decision-revision-detector — merge delta specs, accept ADR-0025 (#1014)

Summary

Archives the decision-revision-detector change. All 191 tasks are checked; Phase B shipped in #1055, #1058–#1061, #1063 and #1065–#1069.

• Delta specs merged into the living specs:
  • forget-command: "Deletion Sweep Includes Persisted Findings" modified.
  • reconcile-command: "Revision Findings Walk" added.
  • decision-revision-detection: a new domain of 16 requirements, written in main-spec shape (# … Specification / ## Purpose / ## Requirements), not as a copied delta.
• The change folder was moved with git mv to openspec/changes/archive/2026-09-29-decision-revision-detector/, including exploration.md, plus a new archive-report.md.
• ADR-0025 is accepted ("Temporal direction between two concepts never comes from a model") in all three places: frontmatter, the body status line and the README index.
• A known, intentional gap is recorded in archive-report.md and in the archived design.md (Decision 8): the two extra revisions stdout lines the design sketched (workspace root; served/judged summary) were never tasked or specified, so they were not built.

A follow-up commit rewrites the new spec's Purpose paragraph. The archive agent had written "Part of MVP 3", which docs/roadmap.md does not support, and the paragraph omitted REAFFIRMS and where the vectors come from.

How was this tested?

• The old change folder is gone, and the archive holds every artifact.
• grep -nE '^## (ADDED|MODIFIED|REMOVED|RENAMED)|Delta for' openspec/specs/*/spec.md is empty.
• Every requirement heading from the three delta specs appears exactly once in its main spec (18 of 18, grep -cxF).
• ruff check ., ruff format --check . and mypy . are clean; pytest --cov 6904 passed, 2 skipped (96.96%), including the ADR status-sync tests.
