fix-heredoc-git-attribution：mask_heredoc_bodies 四态遮蔽（引号数据全遮蔽/ 无引号保留替换跨度/Shell 解释器不遮蔽/非 Shell 全遮蔽），遮蔽先于替换展开； split_shell_segments 与 _flatten_substitutions 双入口接线。 五态语义回归 12/12；无引号 $(git) 向量与 bash 建模保持不变。
