统一外部进程执行器增加 stdout/stderr 合计 16 MiB 捕获预算；超限停止执行，保留部分诊断并显式返回 UNVERIFIED，避免长输出耗尽内存或被误判为 PASS。维持正常输出、argv、超时和旧 tuple 协议；规格与架构文档同步。验证：549 项单测、143 项真实 Hook 回归、Ruff、架构检查、语言 schema、vendor 离线/在线、OpenSpec strict 均通过。Windows 子孙进程、Git 二进制快照及三宿主已安装运行仍需独立验证。
