unlist() prefixed operation ids with the chunk index, so ops fetched in more than one chunk were stored under the wrong id and recorded as missing in the edit cache. Existing caches built this way should be deleted. Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>

Adds a vignette plotting the history of human proofreading edits (cumulative and per week, merges vs splits), and fixes two bugs found while building it.
