v4.1.0 を確定する。v4.0.0（2026-09-28）以降に main へ入った 41 本の Pull Request（スプリント m02-ci・m03-names・m7a〜m7e と確定仕様化・AGENTS.md の改訂）の CHANGELOG を確定する。
