V2 Phase 0: Repository audit — CURRENT_STATE.md + check in CORTEX_V2.md

Background

Cortex V2 plan (vNext). Phase 0 must complete before feature work.

Problem Statement

No authoritative gap report mapping V2 enhancements to what is fully implemented vs partial vs docs-only.

Proposed Solution

1. Copy V2 spec into docs/CORTEX_V2.md
2. Produce docs/CURRENT_STATE.md covering:
   • fully / partially / docs-only capabilities
   • README claim mismatches
   • reuse points (models, migrations, scorers, APIs)
   • duplicate/inconsistent logic
   • recommended module placement
3. No architecture or feature code changes in this issue

Acceptance Criteria

• docs/CORTEX_V2.md in repo
• docs/CURRENT_STATE.md complete per V2 §17 Phase 0
• DECISIONS.md / CLAUDE.md note V2 Phase 0 complete
• No feature code in the PR
