Phase 2 only. Replace raw commit-count scoring with cadence-aware activity scoring, replace issue closed/open ratio with stale-issue and resolution signals, and replace average PR merge time with median/p90 merge time plus stale-open-PR signals. Adds deterministic unit tests.
