This commit only removes the missing billing screenshot and states the monthly equivalent of each list price in the rate-card table. The arithmetic checks out; two previously raised defects are still present at this head.

• Rate-card table now carries a per-month figure (or a note) beside each list price
• The Maple section's Settings → Billing usage screenshot is gone; no broken image left
