Rename GitNexus index refs to graycode-skills - #41

The GitNexus index name is derived from the repo name at analyze time. Since the repo was renamed to graycode-skills, the rebuilt index is graycode-skills; update the gitnexus://repo/... refs and index-name references in AGENTS.md and CLAUDE.md to match.
