feat: Claude issue triage workflow for Tetris issues - #5

On issue opened/edited (title or body) or when the `tetris` label is added, Claude analyzes the issue, applies existing labels (type, priority, area) and posts a single, updatable diagnosis comment with root cause, affected code and a suggested implementation approach.
