SEP-2260: Require Server requests to be associated with a Client request

Tracking implementation of SEP-2260 for the 2026-07-28 MCP spec release.

The Java SDK side does not allow elicitation, sampling or roots listing to happen outside of a request handler - there is no API for that. We are already covered.
The client does not discriminate whether an elicitation request was made during a request or if it was unsolicited, and does not throw the recommended error.

For the record, the Typescript SDK considers that the "no server-initiated requests" part of the spec is handled structurally through Multi Round-Trip Requests (#1004) .
They plan to implement some link between outbound client request and inbound elicitation. See modelcontextprotocol/typescript-sdk#2186 for more context.

This means that we may already cover this spec. Let's verify once we implement MRTR
